<title>AAOS Commends CMS for Important Changes to Bundled Payment Models</title>

<p style="margin: 0in 0in 0.0001pt;"><b><span>Washington, DC--</span></b><span>Yesterday, the Centers for Medicare &amp; Medicaid Services (CMS) announced a proposed rule that addresses significant concerns raised by the American Association of Orthopaedic Surgeons (AAOS) related to mandatory bundled payment programs. First, the proposed rule would reduce the number of mandatory geographic areas participating in the Center for Medicare and Medicaid Innovation’s Comprehensive Care for Joint Replacement (CJR) model from 67 to 34. In addition, the proposed rule would allow CJR participants in the 33 remaining areas to participate on a voluntary basis. CMS also proposes to make participation in the CJR model voluntary for ALL low volume and rural hospitals in all of the CJR geographic areas. Finally, CMS is proposing to cancel the Surgical Hip and Femur Fracture Treatment (SHFFT) payment model and others that were scheduled to begin on January 1, 2018.</span></p>
<p style="margin: 0in 0in 0.0001pt;"><span>&nbsp;</span></p>
<p style="margin: 0in 0in 0.0001pt;"><span>AAOS commends CMS for this important proposal and, after further analyzing all details in the rule, AAOS will be submitting comments by the October 16 due date.</span></p>
<p style="margin: 0in 0in 0.0001pt;"><span>&nbsp;</span></p>
<p style="margin: 0in 0in 0.0001pt;"><span>"AAOS applauds Secretary Price, Administrator Seema Verma, and others at CMS for clearly hearing concerns of orthopaedic surgeons related to these mandatory payment models," stated AAOS President William J. Maloney, MD. "As we have said before, AAOS strongly supports the efforts of all stakeholders to develop payment models that incentivize care coordination and address rising health care costs. Additionally, appropriate alternative payment models are a necessary component of the current Quality Payment Program. However, imposing mandatory models on surgeons and facilities that lack the familiarity, experience, or infrastructure required has serious unintended consequences. Reducing the geographic area for CJR while still leaving a voluntary option significantly remedies this issue. We thank CMS for their work on this proposed rule and will be commenting officially with a more detailed response."</span></p>

<p style="margin: 0in 0in 0.0001pt;"><span>“Changing the scope of these models allows CMS to test and evaluate improvements in care processes that will improve quality, reduce costs, and ease burdens on hospitals,” said CMS Administrator Seema Verma. “Stakeholders have asked for more input on the design of these models. These changes make this possible and give CMS maximum flexibility to test other episode-based models that will bring about innovation and provide better care for Medicare beneficiaries.”</span></p>

<description><![CDATA[<p class="">HOUSE BACKS CHANGES FOR SURGICAL, RECOVERY CENTERS</p>
<p  class=""> The House on Wednesday easily approved a bill that would allow patients to stay longer at ambulatory-surgical centers and open the door for a new type of health-care facility known as "recovery care centers." The bill (HB 85), sponsored by Rep. Heather Fitzenhagen, R-Fort Myers, is part of a series of health-care regulatory changes backed by House Republican leaders. Currently, patients cannot stay overnight at ambulatory-surgical centers, but the bill would allow them to stay up to 24 hours. Also, the bill would allow the creation of recovery care centers, where patients could stay up to 72 hours after surgery. House members voted 82-34 to approve the bill, which now goes to the Senate. Fitzenhagen said the bill can help lower costs and increase treatment options for patients. It remains unclear, however, whether the House and Senate will reach agreement on the issues before the&nbsp;March 11&nbsp;end of the legislative session. The Senate has included the ambulatory surgical-center proposal in a much-broader bill (SB 212) dealing with health-care issues.<br>

<span style="color: rgb(0, 0, 0);">Last week, the House Energy and Commerce Health Subcommittee held a two-day hearing on the need to advance a permanent legislative solution to the "broken Medicare physician reimbursement formula" known as the&nbsp;</span><strong style="color: rgb(0, 0, 0);">Sustainable Growth Rate (SGR)</strong><span style="color: rgb(0, 0, 0);">. The hearing, entitled "A Permanent Solution to the SGR: The Time Is Now," was the first of the 114th Congress on this issue and comes about two months before the current patch expires. Though the last Congress made significant progress on the SGR policy, political realities - namely, how to pay for the bill - stalled final passage so that a one-year patch was necessarily enacted instead. At last week's hearing, legislators again expressed a strong commitment to fixing the SGR, which has plagued the health care industry for over a decade.</span><br style="color: rgb(0, 0, 0);">

<span style="color: rgb(0, 0, 0);">"This Subcommittee has made permanent repeal of the flawed Medicare Sustainable Growth Rate formula, or SGR, a top priority for the last four years," stated Health Subcommittee Chairman Joseph Pitts (R-PA). "In 2014, we reached a bipartisan, bicameral agreement on a replacement policy that enjoys widespread support both in Congress and among the stakeholder community. With the current "doc fix" expiring in less than two months, at the end of March, we are faced with the best opportunity in a decade to permanently dispose of the SGR. We are committed to rising to meet this challenge."</span><br style="color: rgb(0, 0, 0);">]]></description>

WCRI does not take positions on the issues it researches; rather it provides objective, reliable information to public policy makers and system stakeholders and serves as a catalyst for change in the system. All WCRI studies are subject to a rigorous peer review process and are published only if the reviewers approve the study as accurate and balanced.&nbsp; WCRI&nbsp;publishes benchmark studies on various aspects of the Florida&nbsp;workers' compensation system. These studies&nbsp;(called <strong>CompScope Benchmarks</strong>) analyze trends&nbsp;and compare&nbsp;Florida with&nbsp;15 other study states.</span></p> <p style="margin: 0in 0in 0.0001pt;"><span style="color: black;">&nbsp;</span></p> <p style="margin: 0in 0in 0.0001pt;"><span style="color: black;">We have a WCRI&nbsp;Florida CompScope™ Advisory Committee&nbsp;that meets to discuss&nbsp;key points in the&nbsp;WCRI draft reports,&nbsp;offer reactions to the preliminary findings,&nbsp;and discuss emerging issues or concerns in&nbsp;Florida.&nbsp;The advisory committee consists of a&nbsp;cross section of stakeholders from labor, business, state agencies, insurance, and&nbsp;health care&nbsp;providers who are knowledgeable about workers' compensation in&nbsp;Florida. </span></p> <p style="margin: 0in 0in 0.0001pt;"><span style="color: black;">&nbsp;</span></p> <p style="margin: 0in 0in 0.0001pt;"><span style="color: black;">We&nbsp;have an opening on this committee for a practicing surgeon, who works with workers' compensation patients.&nbsp;We have medical providers who represent other fields of medicine treating workers' compensation patients appointed to the committee and I would appreciate your help in identifying a candidate to fill the vacancy we have in this discipline. &nbsp;</span></p> <p style="margin: 0in 0in 0.0001pt;"><span style="color: black;">&nbsp;</span></p> <p style="margin: 0in 0in 0.0001pt;"><span style="color: black;">The&nbsp;time commitment is modest. The committee meets twice each year usually in Tallahassee for approximately 3 hours.&nbsp; The&nbsp;next&nbsp;meeting will be in January or February 2014 and focus on the key points from the&nbsp;CompScope Medical Benchmarks, which drill down into the medical costs and utilization for&nbsp;Florida. </span></p> <p style="margin: 0in 0in 0.0001pt;"><span style="color: black;">&nbsp;</span></p> <p style="margin: 0in 0in 0.0001pt;"><span style="color: black;">WCRI is a nonprofit, nonpartisan research organization.&nbsp;We do not take positions on the issues we research.&nbsp; We do no advocacy work or lobbying.
